If this is your first appointment, please arrive 10 minutes early to complete a new patient questionnaire. To save time, this form can be downloaded here and returned prior to your appointment.

Dr O’Neill can arrange Telehealth appointments when needed for initial consultations in certain circumstances. Please contact to discuss.

What should I bring to my initial appointment?

A current referral from your GP or Specialist 

  • New Patient Registration Form (To save time, this form can be downloaded here and returned prior to your visit, or you can complete it when you come in.)

  • Medicare, DVA and Pension cards

  • Your Private Health Insurance details

  • Any tests or scans, including blood tests, ultrasounds, X-rays, MRIs or CTs you have had done

  • A list of all medications you are on (including prescription, vitamins, supplement or herbal medicines)

  • Any relevant correspondence from other specialists who may be involved in your care

  • List of any known allergies

  • Glasses (if you wear them)

  • A list of any questions you may wish to ask so you don’t forget on the day

You are welcome to bring a support person to your appointment if you wish to do so.

Post-Operative Appointments

If you undergo surgery, the necessary post-operative appointments will be scheduled for you. These are important appointments to monitor your recovery progress and avoid complications.

Fees & Payments

  • Private Health Funds do not provide rebates for specialist consultations. There will be a fee for the consultation and we would appreciate if you could pay that on the day via eftpos (or cash). You will receive a rebate from Medicare if eligible and you have a current referral. This can be processed for you at the time of consultation.

  •  Unfortunately Dr O’Neill does not see uninsured patients at this time.

Cancellations

  • If you cannot attend your appointment, please contact the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list. Alternatively, if you are unwell, in some circumstances we can change to a telehealth or phone consultation. Please call the office to discuss.

  • Same-day cancellations may attract a cancellation fee, which would need to be paid before further consultations are offered.

  • We recognize that your time is valuable and make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.

Do you offer Bulk-Billing?

No, Dr Sarah O’Neill operates a private practice clinic. Therefore, patients will incur an out-of-pocket expense associated with most consultations and procedures.

Do I need a GP referral?

Yes, having a GP referral allows you to receive the Medicare rebate and allows your GP to send a referral letter with your full medical history detailed, which is essential for your urologist to review.

Do I receive the Medicare rebate?

If you are a Medicare card holder and have a valid referral, the Medicare rebate can be claimed directly at the time of your appointment payment.

How long is a referral valid for?

GP referrals are valid for 12-months and referrals from other Specialist are valid for 3-months.

The referral starts from the date the specialist first meets the patient, not the date issued.
